DESCRIPTION OF BENEFIT
While you have the freedom to choose any healthcare provider, Beech Street can direct you to their in-network providers, which will reduce your in-network costs regardless of your eligibility for Insured benefits.
HOW TO USE THIS BENEFIT
- Prior to hospital admission:
- Contact Member Services at (800)275-1171, Monday though Friday, 8 a.m. to 5:30 p.m., Central Time. You will be provided with a list of participating providers in your area.
- Go online at www.iabbenefits.com
- Call the participating hospital or medical facility to set up an appointment and/or admission.
- Present your membership card to the hospital or medical facility at the time of admission.
- You and the hospital or medical facility will receive a statement of savings indicating any amounts due to the hospital or medical facility. Payment should be made within 10 days of receipt of statement to avoid loss of savings.
- If applicable, submit a claim for hospital or medical facility visit. Call (800)275-1171 to get a claim form.
